Appointment Preparation Checklist (Printable)

Use this checklist to get the logistics of an appointment sorted ahead of time, so the day itself is calmer. Check items off on screen or print a copy for each visit.

How to use this checklist

The checklist below covers the logistics around an appointment, not the appointment itself. It is grouped by timing: a few days before, the day before, on the day, and afterward. It works for a doctor's visit, a dental cleaning, a meeting at a bank or benefits office, or a haircut that needs a ride.

Your check marks are saved in this browser on this device, so you can close the page and come back. Many families find it easier to print a fresh copy for each appointment and clip it to the front of a folder that travels with the person.

Plan it with the person, not for them

If you are helping an older relative, go over the list together. They know which questions matter to them, which time of day suits them, and whether they want company in the room or only a ride to the door. Ask before you call on their behalf, and let them decide who hears about the visit afterward.

A few days before

Most of the work happens here. Confirming the time and address early gives you room to fix a mix-up, such as an office that has moved or a visit that was booked for the wrong day. It is also the time to arrange a ride. If a family member is driving, write down who it is and a backup name. If you are using a community ride service, ask how far ahead they need a booking and how long the pickup window is. The guide to planning transportation lists questions to ask.

Writing questions down before the visit helps them get asked. Keep them short and in order of importance, so the most important one gets asked first if time runs short.

If the office has asked the person to bring a list of their current medicines, gather that list as they requested. Zeitlos does not give advice about medicines; questions about them belong with the person's health care provider or pharmacist.

The day before

This group is a quick check: confirm the ride, put everything in one bag by the door, charge the phone, and look at the weather. Setting out clothes and a snack the night before can make the morning less rushed. If the appointment is early, plan the wake-up time backward from the pickup time.

On the day

Leave extra time for parking, finding the right building, and checking in. Bring something to read or do while waiting. If you are going along, agree beforehand on your role: taking notes, asking a question the person wants help with, or simply waiting outside.

Afterward

  1. Write down the next date. Put the next appointment on the household calendar or the weekly home planner before the reminder card gets lost.
  2. Share logistics, not details. Tell the family what they need to know to help, such as the next date and whether a ride is needed. Health details belong to the person and are theirs to share.
  3. Keep private notes private. Store any personal notes in a folder or place the person chooses, not on a shared board or group chat.

The Family Responsibility Planner can help you record who handles rides and scheduling. It is for logistics only; please do not enter health information into it. For a broader system, see the guide to organizing appointments.

Common questions

Is this checklist only for medical appointments?

No. It covers logistics such as confirming the time, arranging a ride, and gathering items, so it also works for dental visits, bank or benefits office meetings, and other errands that need planning.

Should I bring a list of medicines?

Bring one only if the person's provider or the office has asked for it. Zeitlos does not give medication advice. Questions about medicines should go to the person's health care provider or pharmacist.

Is my progress saved?

Yes. Check marks are saved in the browser on this device. They are not sent to Zeitlos. Print a copy if you want to bring it along or share it.

What should I share with family after the visit?

Share the logistics they need to help, such as the next appointment date and whether a ride is needed. Health details belong to the person, who can decide what to share and with whom.
